need to reach the physician after hours or would like to leave a
message for the office staff to call you back in the morning, please
call the office (650) 348-1242 and listen for further information.
About the Office
Our office is
located in Historic Downtown San Mateo, just off of El Camino Real.
Parking is available in our building and validated at the time of
your visit. If you are a new patient, please arrive 15 minutes prior
to your visit.
Fees, Insurance and Billing:
Dr. Wolf is a
participating provider for Medicare and several other insurance
carriers listed below. As a service to our patients we will direct
bill your insurance carriers for your visits, injections and any
medical in office procedures, if those services are a covered
benefit by your health care plan. To successfully bill on your
behalf, we need accurate, complete insurance information from you.
Most of it appears on your insurance identification card, but
sometimes additional information may be needed. Please keep in mind
that the contract for payment coverage is really between you and
your insurance carriers. Insurance identification card must be
presented at the time of your visit. Office visit co-payment is
required and can be paid with check, cash, Visa and MasterCard for
your convenience.
